We're so glad you've decided to fill out our Membership Application
and become a member of our Association!

All you have to do now is fill in ALL the blank spaces and answer ALL the questions,
and send it in to us.

We would appreciate it if you would please answer a few questions below. We are gathering this data so that we may more fully and comprehensively help people to plan for a secure financial future. From the information you provide on this application, we will be able to ascertain what areas we need to focus on to better help you, as well as others seeking financial security.

And, of course, we need the personal information on you to confirm your eligibility
and to contact you with details about our Association.

(Due to a law recently passed regarding the protection of minors on the internet, we must ask for birthdate/age to confirm that you are over the age of 18 years old. While we believe that people should start learning how to save at an early age, this law makes it prohibitive to collect any information from a minor. If you are a parent, we would recommend you share the knowledge gained from being a member of our Association with your children.)

It makes no difference if you answer the questions correctly or not.
By filling out this form and sending it to us you are requesting and receiving free membership
into the Horizons Financial Planning Association, and as a member will receive valuable information
that will help you to plan for your future and secure your financial horizons.
